Referrers can securely submit patient details using the online referral form below. To assist our intake team, please include relevant clinical notes, current medications, and any previous assessments.

Each referral is carefully reviewed by our clinical team, and patients are contacted directly. We value collaboration and maintain ongoing communication with referrers to ensure continuity of care and a coordinated treatment approach.

If your patient has not received an Assessment and Management Report (Item 291) within the past 12 months, please clearly state in your referral that you are referring for an Assessment and Management Report (Item 291) to ensure the patient can access the appropriate Medicare rebates.
